All forms must be completed by June 15. You can scan and email them to us if you are going to miss the meeting.
Meeting will take place Tuesday - June 15 (High School Boys Teams will be held in October)
All Team Olympic Boys must complete these forms:
1. Player Registration Form for YSSL
2. Communicable Disease Liability Waiver
3. Player Information
4. Parent/Player Code of Conduct (2 pages)
5. Team Olympic Liability Waiver & Medical Release Form
6. IYSA Player Medical Release & Liability Waiver
7. Payment Plan Commitment for U14 and younger - CLICK HERE. For High School Boys - Click Here
8. Please also take a head shot photo of player and email to us if you will not attend the meeting.
